How to Order a Replacement Certificate
1. Send $20 (cash, cashiers check or money order) and a self-addressed stamped envelope to:
IF YOU SEND A PERSONAL CHECK, THE CERTIFICATE WILL BE HELD FOR THREE (3) WEEKS UNTIL THE CHECK CLEARS! TO HAVE THE CERTIFICATE SENT QUICKER, THEN YOU MUST SEND EITHER CASH, A CASHIERS CHECK OR A MONEY ORDER.
Diamond Driving School 9109 Veterans Dr. SW Lakewood, WA 984982. Include a letter requesting a replacement certificate. The following information will speed up the process. Include: Location where the student attended class, dates of the class as well as the times of the class. (If you have approximate times and dates, that will be ok!)
3. Finally, include your name, the name of the student, the students permit number, the students date of birth, as well as the phone number where you can be reached.
